Before a license will be issued a copy of the Worker's Compensation Insurance must be attached to this application if you have five (5) or more Employees. Any construction company with (1) or more employees must provide a copy of Worker's Compensation Insurance.

An application fee of $25 must be paid prior to this application being processed.
